Using a multidisciplinary approach and our model of family-centered care, the rehabilitation team works together to provide the best care and communication for maximum functional outcomes. The rehab staff, patients and families interact often during the patients stay on the unit.
The team meets daily in morning rounds and two times a week in patient care conference to discuss patient updates, functional status, and discharge disposition. Additionally, the rehab staff collaborates regularly with patients, families and each other on an informal basis in order to most efficiently progress the patient toward his goals.
